The Partners
US Partners: The initial US Partner was
Beth Israel Hospital of Boston, Massachusetts. In April 1995, Beth Israel withdrew from the partnership, agreeing to continue work in the focused area of women's reproductive health through self-financing and the Charitable Trust for Women's and Children's Health in Armenia, a Boston-based 501C3 foundation.
UCLA Medical Center joined the partnership in 1995. The Medical Center, with more than 600 beds, offers comprehensive care to patients of all ages, from routine to highly specialized medical and surgical treatment. The Medical Center includes specialized intensive care units, state-of-the-art inpatient and outpatient operating suites, a Level-1 trauma center, the latest diagnostic technology, and an experienced staff of over 1,000 physicians and 3,500 nurses, therapists, technologists and support personnel.
NIS Partners: Erebuni Hospital, a 900-bed institution, is active in women's health, neonatology, and as of 1995, the school of nursing. The Ministry of Health is responsible for the development of national health care policy. It is a strong supporter of the partnership, and the director of Erebuni Hospital is a member of the Ministry's health services committee.
Partnership Objectives
The partnership had three primary areas of focus:
Women's Health
- Design and open a satellite women's health center based on the AIHA Women's Wellness Center model.
- Support regional conferences on maternal and child health.
- Create a patient education center at the Erebuni Women's Health Center.
- Increase knowledge and use of contraceptives.
- Conduct training for Erebuni staff in areas of colposcopy, gynecological laparascopic surgery, breast and cervical cancer screening, cyto-pathology, endocrinology, high-risk obstetrics and other ob/gyn procedures and preventive health practices.
- Design and implement a Lamaze childbirth program.
- Develop a business plan, incorporating a fee-for-service cost structure at the Erebuni Women's Health Center.
- To disseminate and further promote the adoption of new standards of primary care for women in additional facilities and regions within Armenia.
- Conduct an outreach program to rural locations of Armenia and conduct related train-the-trainer sessions.
Neonatal Resuscitation (added in 1995)
- Conduct an initial needs' assessment for the Neonatal Care Unit staff.
- Open a neonatal resuscitation center at Erebuni Hospital and offer neonatal resuscitation training courses.
- Develop and implement standards of care for the Neonatal Intensive Care Unit.
- Decrease the mortality rate in the Neonatal Intensive Care Unit.
- Increase the role nurses play in providing neonatal care.
Nursing Education and Care
- Develop and implement a Baccalaureate Degree Program in Nursing.
- Establish an Armenian Nursing Association, and assist the Association in conducting international nursing conferences.
- Establish a Nursing Learning Resource Center and develop a continuing education training program for nurses.
- Develop a qualified cadre of nurse faculty for the School of Nursing.
- Provide opportunities for advanced nursing practice.
- Provide train-the-trainer workshops for nurses participating in AIHA's International Nursing Leadership Institute (INLI).
- Produce and disseminate training curricula in Armenian language, based on the INLI curriculum.
Key Events
1992
- Yerevan partners opened the Women's Health Center at Erebuni Medical Center, offering free clinics every Friday and every other Monday.
1993
- In May, partners celebrated the signing of the Memorandum of Understanding between Beth Israel Hospital in the US, and Erebouni Medical Center in Armenia.
- The Armenian partners developed and launched a fellowship program for physician trainees in the summer of this year.
1994
- The Yerevan partners, in collaboration with Harvard University, established an iodine deficiency program in the summer of this year.
- A rotating fellowship program was instituted in 1994, which trained cytologists and ultrasound physicians from the 3rd Maternity Hospital and the Railway/Military Hospital at the Women's Health Center at the Women's Health Center.
1995
- Erebuni Hospital developed protocols for gynecology and obstetrics in March.
- In June, Erebuni Medical Center signed a Memorandum of Understanding with their new US partner, UCLA Medical Center.
- The Armenian partners opened a Neonatal Resuscitation Center in September.
- The Yerevan partners established the Baccalaureate Degree Program in Nursing, the first higher education program for nurses in Armenia, in September.
- Erebuni Hospital implemented a computerized medical records system in conjunction with Arminco. The system allows hospital staff to make daily entries of medical history, chief complaint, diagnosis, and clinical management.
1996
- In April, the Yerevan partners formed the Armenian Nurses Association. In the same month, the partners held the 1st Armenian Nurses Association Conference.
1997
- Over 200 nurses participated in the 2nd National Armenian Nurses Association Conference in June, the Association's first international event.
- Erebuni Hospital opened a patient education center in June.
- The partners celebrated the opening of the Nursing Resource Center in June.
1998
- On May 12, partners opened a satellite women's health clinic as part of the AIHA-supported Women's Wellness project. The satellite clinic is located in downtown Yerevan and is easily accessible to women.
- The Armenian Nurses Association held its 3rd annual conference with over 100 participants from all regions of Armenia attending. This was the first conference planned and executed by Armenian nurses, with American colleagues providing support and feedback. For the first time in Armenia's history, a nursing conference was held in the auditorium of the Ministry of Health, demonstrating the Ministry's commitment to the profession and the elevation of nurses' status.
1999
- In July, Erebuni College of Nursing graduated its first class of 18 BSN nurses in a ceremony attended by over 700 people at the American University of Armenia attended. Ten of the eighteen students graduated with honors and were inducted into Sigma Theta Tau, the international nursing honor society. Each graduate received a letter from the California Board of Registered Nursing stating that they were eligible to apply for the Board Exam in California. The Minister of Health, in recognition of these students' achievements, granted them "nurse manager" certificates.
Achievements
Women's Health
- The Erebuni Women's Health Center was established in 1992 with the help of Beth Israel, Harvard Medical Alumni, the Charitable Trust for Women's and Children's Healthcare in Armenia (Boston), and the Armenian community of Boston. Later, the UCLA Medical Center played a key role in the continued development of the Center. Currently, the Center provides a wide range of state-of-the-art OB/GYN services, including contraception, prenatal screening, delivery techniques and Lamaze, ultrasound, colposcopy, laparoscopy, pathology, STDs, and adolescent education. There has been extensive training of Erebuni staff in prenatal, delivery and postnatal care.
- The Center's results have been impressive. Diagnostic ultrasound reduced perinatal mortality rates from 26 per 1,000 births in 1994 to 10.5 per 1,000 births 2000. A referral system was established with the cardiac center and protocols have been introduced for improving the care of the mother and child. The Center provides the latest cytology and pathology testing. Since 1992, the Center has provided 93,450 PAP smears, where 28% were first time clients and 72% came for repeat exams. Of those tested, 17% were identified as having dysplasia or cancer.
- Erebuni Hospital introduced a broad range of contraceptive services as a result of partnership involvement. Since it opened, the Yerevan WWC has served 19,050 clients, primarily in the age range of 20-35 years of age. Of these, 12,019 were seen for medical problems, 3,706 for preventive health exams, and 3,325 for family planning counseling and contraceptives.
- The Center provides education and counseling on the ill effects of abortion. During the period since the WWC opened, the crude abortion rate in the Center's catchment area fell by 21%.
- After the introduction of improved prenatal care and the neonatal resuscitation program, maternal and infant deaths (combined) have been reduced by 60%.
- Anecdotal evidence indicates the number of patients with STDs has dramatically declined.
- Finally, due to a more accurate diagnosis of cervical abnormalities and the introduction of the LEEP treatment method, there has been a reduction in scarring and cervical functionality in women treated at the Center.
- The Beth Israel partners supported several Armenian and English publications, including a brochure on abortions, contraceptive management, a book on baby care, a patient assessment form, a women's health manual, and teaching guidelines.
- The Beth Israel partners assisted the staff of the Erebuni Women's Center to develop a fee-for-service system which includes a sliding-scale for payment and free service days for victims of war, widows, unemployed and other vulnerable patients.
- The Beth Israel partners donated nearly $5 million in product, much of it in-kind, some in cash from fund raising activities in the Boston area. Equipment and supplies donated include contraceptives and reagents as well as medical equipment such as a colposcopic microscope, ultrasound machines, and a complete laparoscopic surgical set.
- To enhance the quality of care offered at the women's center and document procedures, the partners developed protocols for the management of high-risk pregnancies, pre-malignant lesions of the cervix, ovarian cysts, amenorrhea, infertility and sexually transmitted diseases.
- The UCLA partners provided additional training in the area of laparoscopic surgeries. In March 1997, a team of surgeons received specific training in Ob/Gyn laparoscopic surgery at UCLA Medical Center. In three months, the laparoscopic surgeries increased from 5% to 90%. To date, trends indicate a decreased rate of infection and hemorrhage compared to conventional surgical approaches.
- The Women's Health Center opened Lam.
