The Partners
US Partner: Coney Island Hospital (CIH) is a 465-bed acute care general hospital, which provides a full range of in-patient and ambulatory services. In addition to an active 24-hour Emergency Division, CIH operates more than 100 outpatient clinics, two satellite outreach clinics and a full-service, Comprehensive Health Clinic in Coney Island.
NIS Partner: The
Odessa Oblast Hospital is a 1500-bed adult referral hospital serving the entire Odessa Oblast. The hospital is the primary referral center for several dozen rural district hospitals and polyclinics in the oblast. In this network leadership capacity, the hospital plays a central role in directing the organization and delivery of care to the oblast. The hospital is also responsible for the continuing education of physicians in all the district hospitals and polyclinic of the oblast and the assurance of standards of care and practice in these institutions.
Partnership Objectives
Neonatology
- Introduce strategies to care for premature, low birth weight and sick neonates.
- Teach infant resuscitation and pediatric infection control and nutrition.
Surgery
- Improve the treatment of hip fractures and other orthopedic surgeries.
- Introduce new techniques and technologies in various types of surgery, including orthopedics, urology, tumor surgery, endoscopy and laparoscopy, and thereby improve the level of care in these areas.
- Introduce the concept of a surgical team, and improve interaction and teamwork between anesthesiologists and surgeons.
- Increase the knowledge of anesthesiologists and operating room nursing staff with regard to all aspects of pre-operative, peri-operative and post-operative patient care.
Infection Control
- Enhance the infection control system at OOH, including basic infection control practices and protocols for monitoring and analyzing infection control patterns in the patient population.
- Improve knowledge of monitoring and electrolyte balance as it relates to infection control.
- Provide the education necessary to establish a hospital-wide infection control program, including the creation of an Infection Control Staff and development of educational literature for all hospital employees.
- Increase the knowledge and resources of the microbiology laboratory as an important element of diagnosing patient infections.
Nursing
- Increase the role, importance and stature of the midwives at Odessa Oblast Hospital (OOH).
Administration
- Update and improve the administrative systems at OOH, including finance, supply and personnel.
- Implement a Quality Improvement Program and establish Quality Assessment Programs for various departments of OOH.
Women's Health
- Establish a comprehensive Women's Wellness Center in a clearly identifiable and designated polyclinic in Odessa Oblast Hospital, based on AIHA's Women's Wellness model. The Wellness Center will provide care for the problems of women starting from the adolescent years to the menopausal years.
- Expand and enhance the radiologic, healthcare professional and surgical staff skills regarding all modalities of breast cancer screening, including mammography, public awareness programs and treatment of patients with breast carcinoma.
- Enhance the education of a designated cytopathologist at the Women's Wellness Center in cytopathology of the breast.
- Improve the detection and proper treatment of non-palpable breast lesions.
- Ensure proper quality control of mammographies as well as the processing technique and equipment maintenance.
Key Events
1992
- On September 2, the partners celebrated the signing of a Memorandum of Understanding between Coney Island Hospital and Odessa Oblast Hospital.
1993
- In September the Coney Island/Odessa and the Philadelphia/Kiev partnerships sponsored a dissemination conference in Kiev for representatives from the six West NIS partnerships. The conference focused on a number of clinical areas common to many of the partnerships including neonatal resuscitation and perinatal asphyxia.
1994
- In May the Coney Island partners presented a surgical seminar for ten of their Ukrainian colleagues.
- The Coney Island/Odessa partnership conducted an international conference during the summer on the most recent advances in specific surgical techniques. Over 200 surgeons from Odessa, other regions of Ukraine, Belarus, Russia, Kyrgyzstan, Moldova, and Armenia attended. Topics included the use of lasers for trophic ulcers and purulent wounds, new techniques in herniotomy using special nets, and laparoscopes for cholecystectomies.
1995
- In September a ten-person delegation of US partners traveled to Odessa to participate with their partners in a two-day conference entitled "Management of Surgical Infections." In conjunction with the conference, a surgical manual and infection control manual were published. Several operations for stomach and colon malignancy were performed. Surgical techniques for abdominal hernia repair were televised and aired in the Odessa Oblast. The magazine Medicine of Ukraine (issue No. 4, 1995) published an article dedicated to this conference.
1996
- A conference on "Leadership in Nursing," which was sponsored by AIHA and the Odessa Oblast Trade Union of Medical Workers, was held in Odessa in June. Over 150 nurses from 26 regions of Ukraine were in attendance. The conference focused upon upgrading the role of nurses in infection control, including both treatment and management.
- The first AIHA-sponsored Nursing Learning Resource Center in the NIS was officially opened in September at OOH. The center provides nursing faculty, students and other practitioners with alternative forms of learning which serve to enhance traditional education methodologies. It also serves as a meeting site for local nursing associations and enables nurses to collaborate on a variety of issues.
- A team of physicians and nurses from the US conducted an infection control hospital survey at OOH in December.
1997
- US surgeons traveled to Odessa in June to participate with their NIS partners in an all-Ukrainian surgical conference, "Laparoscopic and Endoscopic Surgery - the Perspectives of the Development and Economic Effects of Implementation." More than 160 medical professionals participated in the conference.
- The head microbiologist from OOH's laboratory received training in the use of the WHONET database, including data collection and antimicrobial surveillance, during AIHA-sponsored workshops in St. Petersburg (June) and in Atlanta (October).
- US and Ukrainian partners conducted the first Neonatal Resuscitation course in Odessa and began operations of the third NRP Center in Ukraine in October.
- The Ukrainian Association of Laparoscopic Specialists, a new professional society, was formed in the spring.
1998
- Neonatologists and health care officials at Odessa Oblast Hospital celebrated the official opening of a Neonatal Resuscitation Training Center (NRTC) in February. Officials from USAID and the Ukrainian Ministry of Health attended the opening ceremony, during which NRTC staff demonstrated the neonatal resuscitation skills to be taught at the center.
- Odessa Oblast Hospital officially opened a comprehensive Women's Wellness/Breast Health Center in November. As with similar AIHA-sponsored Women's Wellness Centers throughout the NIS, the Odessa center will provide family planning services, screening for sexually transmitted diseases, education and other health services for women from adolescence through menopause. The Odessa center will also focus on breast cancer education and screening, including mammography.
Achievements
Infection Control
- An Infection Control Committee was created at the Odessa Oblast Hospital (OOH) early in 1996; the committee meets monthly to discuss infection control related issues. All heads of hospital units are required to report all cases of infection complications to the hospital Infection Control Committee.
- Within the framework of Infection Control Program, the senior duty nurse is responsible for supervising both nurses and physicians in respect to compliance with Infection Control regulations.
- In May 1996 the positions of epidemiologist and deputy epidemiologist were created at OOH, including duties and responsibilities for each position.
- The OOH has implemented rational antibiotic therapy protocols.
- As a result of basic and advanced infection control training, OOH reported a reduction of surgical site infection rates by 35 percent.
- Since the inception of the infection control program, the occurrence of post-operative infectious complications decreased by 50 percent and the post-operative average length of stay was decreased by 3.5 days. The chart below demonstrates the decline of the average length of stay and other surgical statistics over the last four years of the partnership.

- As a result of the strides made in the area of infection control at the OOH, AIHA and the developers of the WHONET program chose OOH as one of six pilot sites. Beginning in July 1997, work began toward upgrading their microbiology laboratory, gathering data on resistance to antimicrobial agents, and, based on the data analysis, implementing mechanisms to minimize the misuse of antimicrobial agents.
- OOH implemented an active focused infection control surveillance program in 1998.
Surgery
- From the inception of the partnership through 1998, over 2,000 laparoscopic procedures have been completed by the surgeons at Odessa Oblast Hospital (OOH).
- Six surgeons from OOH have become experts in laparoscopy, and laparoscopic cholecystectomy is now performed as a standard procedure in 73 percent of patients with cholecystitis. Since the inception of the program over 1,300 laparoscopic cholecystectomies have been performed. As a result of all the improved surgical techniques, there has been a reduction in average length of stay for cholecystectomies from 11 days to 3.8 days.
- The OOH was the first hospital in Ukraine to perform laparoscopic vagatomy.
- The hospital-wide reduction in the length of stay has been reduced from 12 days to 5 days and the average length of stay in the surgical department has declined from 14 to 10.5 days.
- As a result of partnership training in new surgical techniques and technologies, hundreds of surgeries have been successfully initiated and performed at OOH. These include laparoscopic and endoscopic surgeries (such as endoscopic papillotomy; ultrasound-controlled hepatic, peritoneal and kidney cystectomy; and balloon gastric dilation); vascular and thoracic surgeries (such as video-controlled lung and pleural thoraxoscopy, and new laser techniques in thoracic surgery); urology surgeries (such as transurethral resection of prostates and internal urethrotomy).
- A new professional society, the Ukrainian Association of Laparoscopic Specialists, was formed in Spring 1997. The Association serves to provide education in laparoscopic procedures to all regional hospitals. The Chairman of the Association is a physician at the Odessa Oblast Hospital who has received extensive training in laparoscopic surgery at Coney Island Hospital.
- The Ukrainian National Association for Endoscopic, Laser and Minimally Invasive Surgery (UNAELMIS) has been established at Odessa Hospital.
Women's Health
- The Head Physician of the Odessa Oblast Hospital created a Mammography Project Technical Group (MPTG) in fall 1997, which is composed of himself, the Director of the Women's Wellness/Breast Care Center, the hospital's chief engineer, senior electrical engineer and senior plumbing engineer, two radiologists, other radiology staff and several US partners.
- The Head Physician from OOH was a member of the committee that created the breast health section of the clinical practice guidelines reference manual being used in each of the AIHA-sponsored Women's Wellness Centers.
- The US and Ukrainian partners have established a process by which the radiologists at OOH will send at least ten mammography films per month (plus other questionable films) to their colleagues at Coney Island Hospital. This system of second reads will serve as a quality assurance program for identification and diagnosis of non-palpable lesions in the breast.
- Partners established a comprehensive Women's Wellness/Breast Health Center in OOH in November 1998. Based on AIHA's Women's Wellness model, the center provides early prenatal care and childbirth classes; breastfeeding education; cancer screening; internal referral for mammography; substance abuse and alcohol abuse prevention education; mental health education, counseling and support; screening education for chronic diseases like hepatitis, hypertension, diabetes, anemia.
Administration
- The director of primary care at OOH implemented a number of administrative and market-reform changes since visiting Coney Island Hospital.
Nursing
- The newly implemented team approach allowed nurses to clearly understand their role in the effective treatment of patients and to begin playing an active role in therapy and treatment. Nurses demonstrate greater interest in the state of their patients as they now view themselves as important and responsible participants in healthcare.
- Approximately 900 nurses received professional training through the seminars, conferences, workshops and meetings sponsored by the partnership. Nearly 2,000 medical professionals have participated in regional conferences sponsored by the partnership.
- The partnership has established an Odessa Oblast Nursing Association at OOH for nurses in the region; the association has approximately 400 members.
Neonatology
- Since the opening of the Odessa Neonatal Resuscitation Training Center in February 1998 through the end of 1998, approximately 450 physicians (ob/gyns, neonatologists, anesthesiologists and pediatricians), nurses and midwives have been trained in neonatal resuscitation.
